Who is eligible for recuperative care at Care Chexx?

Adult individuals experiencing homelessness who are being discharged from a hospital, emergency room, or a clinic but still require short-term medical recovery support are eligible for our services. Referrals typically come from hospitals, healthcare providers, and public health clinics. 

Referrals must be ambulatory and connected to Minnesota benefits in MN-ITs.  (MA, NM, RM, IM, EH, KK, LL (Ages 19 or 20), FF, BB, XX, OO, and most HPE cases also known as ("MA Pending")

How long can a patient stay at Care Chexx?

Up to 21 days but may be extended up to a maximum of 60 days. The length of stay varies based on each individual’s medical needs and recovery progress. Our goal is to provide a safe and supportive environment until the patient is well enough to transition to stable housing or other appropriate care.

Do you provide medical treatment at your facility?

We do not offer direct medical treatment, but we provide medical oversight through partnerships with home health services, visiting nurses, and healthcare professionals to ensure our clients receive the care they need.

How is Care Chexx different from a shelter?

Unlike traditional shelters, Care Chexx offers a structured environment with medical oversight, case management, and supportive services designed specifically for individuals recovering from illness or injury. We focus on stabilization and recovery rather than just providing temporary housing.

How can hospitals and service providers refer patients to Care Chexx?

Healthcare representatives can complete our online referral form, upload their hospital or clinic form, or contact our intake team for referral guidelines and availability.
