Section 1: Recuperative Care Overview
Care Chexx Recuperative Care provides short-term, medically supportive housing for homeless adults who need stabilization after hospital discharge or referral from a clinic. The program helps individuals recover safely in a structured environment, offering private rooms, daily meals, medication monitoring, vital sign checks, and health education.
Supportive services may include social work, transportation to follow-up appointments, connection to housing resources, and assistance with employment or benefits. The goal is to prevent hospital readmissions, support recovery, and transition individuals toward stable housing and long-term health management.
Services include:
- Safe private recovery environment
- Daily wellness checks
- Medication monitoring
- Health education
- Transportation coordination
- Case management support
- Discharge planning and housing navigation
Care Chexx LLC provides services without discrimination based on race, color, national origin, religion, sex, gender identity, sexual orientation, age, disability, or any other protected status. Clients have the right to file a grievance if they believe they have been treated unfairly or denied services.

Section 2: Medication Monitoring & Health Education
While in recuperative care, clients play an active role in their recovery. Responsibilities include:
- Medication Monitoring & Health Education: Take medications as prescribed and participate in health education sessions to understand their condition and care plan.
- Daily Vital Checks: Cooperate with staff during daily checks of vital signs and report any new symptoms or concerns.
- Care Coordination & Follow-Up Appointments: Attend all scheduled follow-up appointments, communicate with social workers or care coordinators, and provide updates on progress.
- Engagement in Recovery: Follow the program’s guidelines, participate in supportive services, and work collaboratively with staff to achieve health and housing goals.
By actively participating, clients help ensure a safe, effective recovery and a smoother transition to stable housing or ongoing care.
Section 3: Daily In-Person Monitoring Guideline
Recuperative care guidelines require that clients are seen in person at least once every 24 hours by Care Chexx staff. This ensures client safety, allows for ongoing monitoring of health status, supports proper medication management, and provides timely interventions if any changes or concerns arise. Daily in-person contact is a key component of maintaining quality care and promoting a safe, effective recovery.
Section 4: Furlough
Any planned absences (furloughs) must be preapproved by the Care Chexx team. Clients are responsible for checking in upon return. Failure to return from an approved furlough without communication may result in program discharge.
Clients may voluntarily leave the program at any time but should notify staff and complete discharge procedures to ensure continuity of care.
Section 6: Involuntary Discharge
Care Chexx may involuntarily discharge clients for safety concerns, violation of program rules, non-compliance with care requirements, or if their whereabouts become unknown.
Section 7: Abandoned Property
Any items left behind by a client after discharge will be considered abandoned. Care Chexx will store items for 30 days before disposal unless other arrangements are made. Care Chexx will release property only to you or your designated emergency contact. Care Chexx will not be responsible for missing or lost items.
Section 8: Discharge Planning
Care Chexx LLC is committed to supporting clients in a safe and successful transition from recuperative care. As part of discharge planning, we work with clients to identify appropriate next steps, including medical follow-up, ongoing care needs, and housing arrangements.
We recognize that clients may choose to be discharged without a housing plan in place. While this is their right, we highly encourage clients to work with Care Chexx on developing a housing plan after discharge to promote stability, safety, and continued health and well-being.

Section 9: HIPAA Authorization for Release of Information
I authorize Care Chexx LLC to use and disclose my protected health information (PHI) as needed for my care and treatment, including communication with insurance companies, healthcare providers, care coordinators, and other professionals involved in my care. This may include information about my medical history, diagnoses, treatment plans, medications, and progress notes. The purpose of this disclosure is to coordinate care, ensure continuity of services, and facilitate billing and payment for services provided.
I understand that this authorization is voluntary, that I may revoke it at any time in writing, and that revocation will not affect any information already shared in reliance on this authorization.
